Die psychoanalytische Selbstpsychologie hat zu einem veränderten Zugang zum Patienten geführt. Grundlage dieser Veränderungen sind im Unterschied zur Triebpsychologie und den Objektbeziehungstheorien andere anthropologische Grundannahmen. Im Mittelpunkt der Behandlung steht der Selbstzustand des Patienten, der ein motivationales Primat vor den Trieben genießt. Unter Berufung auf die Beobachtungsmethode der Empathie und Introspektion wird der subjektiven Erfahrung des Patienten ein sehr viel größerer Spielraum eingeräumt. Es entsteht ein anderes Übertragungsverständnis (Selbstobjektübertragungen) und eine veränderte Schwerpunktsetzung in der therapeutischen Technik. Innerhalb der Selbstpsychologie vertreten Anna und Paul Ornstein die ursprüngliche, eng an Kohut angelehnte sogenannte klassische Selbstpsychologie. In diesem Buch sind erstmals ihre wesentlichen Arbeiten zur klinischen Praxis der psychoanalytischen Selbstpsychologie zusammengestellt. Den Mittelpunkt ihres therapeutischen Umgangs bildet die entscheidende Rolle des empathischen Zuhörens und die Bedeutung der subjektiven Erfahrung des Patienten. Die grundlegenden Schritte des interpretativen Prozesses in der Selbstpsychologie, nämlich Verstehen und Erklären, werden in ihrer Abhängigkeit von der Methode der Datensammlung dargestellt und im Hinblick auf daraus folgende Konsequenzen für die Wirksamkeit im therapeutischen Prozess beleuchtet. Dabei liegt der Fokus auf den Selbstobjektübertragungen des Patienten, seine in der Behandlung erfahrenen, die Selbstkohärenz fördernde Beziehung zum Psychotherapeuten bzw. Psychoanalytiker. Unter diesen Gesichtspunkten erhalten gängige psychoanalytische Begrifflichkeiten eine etwas andere Bedeutung. Statt von Wiederholungszwang wird von der Furcht vor Wiederholung gesprochen und darauf hingewiesen, dass in jeder Wiederholung die Angst vor dem bekannten Ausgang und das Bedürfnis nach einer neuen, dem Selbstzustand angemesseneren Erfahrung enthalten ist.
