Abstract: Abstract of thesis entitled Experiences and Meaning Reconstruction among Chinese Women with Breast Cancer in Hong Kong Submitted by LEUNG Pui Yu, Pamela for the degree of Doctor of Philosophy at The University of Hong Kong in December 2007 Background: Cancer diagnosis can be traumatic and devastating for some patients. Increasingly, studies have been done to explore the relationship of meaning reconstruction and adjustment to cancer. Yet, documentation of the experiences among the Asian population is limited. Objectives: This study aims to understand Chinese women's subjective experiences in adjustment to cancer in meaning reconstruction and to explore the cultural features associated with it. Methodology: Taking a constructivist grounded theory approach, theoretical sampling was used to recruit twenty-six Chinese women with breast cancer aged from 25 to 69. The researcher develops an innovative, multi-method approach which integrates life review interview with drawing of autobiographical timeline and an experiential exercise of visual imagery to capture experience-near information. A rich pool of data in the form of narratives, sketches of autobiographical timelines, and metaphors on conceptions of self was collected. The narrative data was analyzed with the help of the QSR NUDIST Nvivo software program. Findings: The experiences of Chinese women after cancer emerged as a five-phase meaning reconstruction process, from deconstruction to reconstruction. These five phases are: (1) experiencing loss and deconstruction of the assumptive world, (2) reappraising and reconstructing the assumptive world, (3) reconstructing selfhood, (4) redefining or reaffirming purpose and priorities in life, and (5) rebuilding or deepening of relationships. This study suggests four patterns of adjustment through this meaning reconstruction process: transformation, recovery, succumbing, and resilience. It proposes that: (a) the transformation and recovery pattern is associated with a process from meaning deconstruction to reconstruction, (b) the succumbing pattern is associated with a process of meaning deconstruction without reconstruction, and (c) the resilience pattern is associated with a process of meaning construction. Cultural beliefs, personal dispositions such as optimism, prior experience of coping with crisis, presence of social support, and a model of resilience are all variables that may affect adjustment. Discussion: Consistent with existing literature, sense making, benefit finding, searching for existential purpose, and identity change were core components of the meaning reconstruction process. Yet, the process was influenced by cultural concepts such as tien-ming, karma, ping-chang-xin and Zen living. The study reveals that beyond recovery, transformation and resilience are possible and resilience is distinct from recovery and transformation. The possible relation between meaning reconstruction and adjustment pattern proposed by this study helps to generate postulations that can be tested in future research. Implications and conclusion: This study expands our understanding of Chinese women's adjustment after cancer and Eastern cultural coping with trauma and loss. It suggests an important role of meaning reconstruction in adjustment to cancer, informs helping professionals of the importance of focusing on strength and meaning as well a
